    CJA is a loving and supportive life-long community for our students and alumni. We are a tuition-free, Catholic lower and middle school for students and families impacted by historic disinvestment. We also accompany alumni as they transition to high school, post-high school options and on to the start of their careers, and we engage with the larger community to support creating access to better educational resources.

    CJA accepts 3rd-5th grade students first come, first served and we are not academically selective. For 6th-8th grade applicants, academics and willingness to partner with the school are taken into account. We accept students in all grades without regard to race, ethnicity or religion.     After students graduate from CJA, they work consistently with their College & Career Persistence (CP) counselor until age 25 and they and their families are considered part of our community forever. Partnering with Families to Create a Healthy Foundation

At CJA, we know that when students are healthy they are better able to thrive in school. We work alongside families, as an added layer of support, to ensure students have access to high quality physical and mental health care and nutritious food.

Meet Nurse Pamela Kiefer

Pamela Kiefer BSN, RN ("Nurse Pam") has been a Nurse for over 30 years with a Bachelor of Science in Nursing from Loyola University and experience working at Evanston Hospital, as a Pulmonary Nurse with NorthShore University Health Systems, with professional athletes of a Chicago professional sports team, and as an independent consultant for pharmaceutical companies developing new medications. She has worked as a School Nurse for over 15 years, with experience working with Pre-Kindergarten through High School & College age students, including Special Education . She is certified by the American Heart Association in CPR and First Aid and is an Illinois Department of Public Health Certified Vision and Hearing Screener. Pam is a member of the National Association of School Nurses and the Illinois State School Nurse Association. She grew up in Naperville and has lived in the Chicagoland area most of her life.

Personalized Student Care

The level of health care access CJA students have is not available at most schools, but the generosity of our benefactor community makes this possible. We are fortunate to have a team of healthcare professionals including a full-time school nurse and a team of full-time social workers who work individually with each one of our students. At the beginning of each school year, Nurse Kiefer also works with every family to create a wellness plan specific to their student’s needs. Plans can include daily care for conditions like asthma or allergies, administering daily medications or conducting a vision or hearing screening. Nurse Kiefer also helps coordinate separate referrals as needed to primary care physicians, eye doctors, dentists, or any additional medical resources students or their families might need.

Community Partner Support

Each school year, Nurse Kiefer coordinates an on-site mobile dental clinic at CJA in partnership with the Children’s Clinic of the Oak Park River Forest Infant Welfare Society. The clinic is available to all students and offers free dental exams, cleanings, sealants and fluoride treatments. We also partner with the Loyola Medicine Pediatric Mobile Health Unit to offer free physicals and vaccinations required by the state of Illinois.

Illinois State Law requires students entering school submit evidence of a complete physical examination and proof of required immunizations. Chicago Jesuit Academy requests students submit paperwork by the first day of school. In addition to the State of Illinois requirements, to keep our community as safe and healthy as possible, CJA also highly recommends all students and staff provide proof of COVID-19 vaccination.

 

The Physical Examination:

 • Must be completed on the State of Illinois Certificate of Child Health Examination Form.

 • Must be dated less than one year prior to the start of school.

 • The HEALTH HISTORY portion on the back of the form must be completed and signed by the parent or the form will not be 

    acceptable.

***All students participating in co-curricular sports activities which compete interscholastically with other schools are required to have a current physical or sports physical on file.

 

The Required Immunizations:

• Diphtheria, Pertussis and Tetanus: A minimum of 3 doses with the last dose after the 4th birthday.  All 6th grade students are

   required to show proof of receiving one dose of Tdap vaccine. (Tetanus, Diphtheria and acellular Pertussis)

• Polio: A minimum of 3 doses, with the last dose after the 4th birthday.

• Measles, Mumps and Rubella: 2 doses, with the first dose given on or after the 1st birthday.

• Hepatitis B: 3 doses with proper spacing between doses.

• Varicella (Chicken Pox): 2 doses, with the first dose given on or after 1st birthday, or verification of disease history

   by health care provider.

• Meningococcal Vaccine Requirements: 1 dose is required for 6th grade.

 

 

The Dental Examination:

 • Proof of Dental Examination is required for all 6th Grade Students by May 15th.

Frequently Asked Questions

When should I keep my child home from school?
  • A fever of 100° F or higher. Keep student home until they are 24 hours fever free without the use of a fever-reducing medicine.
  • Vomiting or diarrhea.
  • Serious, sustained coughing, shortness of breath or difficulty breathing.
  • New rash accompanied by a fever. May return after rash resolves or the student is cleared by a healthcare provider.
  • If a student is prescribed antibiotics for a contagious disease, the student may return to school after they have been on the antibiotic for 24 hours.
  • Symptoms that prevent a student from participating in school activities or student is requiring more care than the school can safely provide.
Who should I notify if my child will be absent from school?

If a student will not be at school for any reason, please call the front desk at 773-638-6103 and press “1” to report an absence.  If no one at CJA is able to answer a parent/guardian’s call, CJA asks that the parent/guardian leave a voice message.  The voice message should include the parent/guardian’s name, the absent child’s name and the reason for the child’s absence.


Promptly reporting a child’s absence to CJA is an essential part of the shared CJA – parent/guardian responsibility to keep every child safe who is entrusted to CJA’s care. 

What if my child gets sick at school?

When a student becomes ill during school hours, the student will be sent to the nurse’s office for evaluation.  If CJA determines that the student has an elevated temperature, gastrointestinal complaints or illness, or symptoms of a communicable illness or rash, a CJA representative will call the student’s parent/guardian and request that the parent/guardian or another approved adult pick up the student as soon as possible. 

A student may not return to CJA until the student has been fever-free for 24 hours without the use of fever-reducing medications, has not had any gastrointestinal illness (vomiting, diarrhea) for 24 hours, and is fit and able to make it through the school day; or has a letter from a physician clearing the student to return to CJA.  A student returning to school following COVID-19 illness or close contact must follow Illinois Department of Public Health Protocol. 

What if my child needs an early dismissal due to a doctor’s appointment?

If a child needs an early dismissal as a result of a doctor’s appointment or a similar obligation that cannot be scheduled for any other time, CJA asks parents/guardians to request the early dismissal at least 24 hours in advance by calling the front desk at 773-638-6103.  CJA asks for advance notice so that the child and the child’s teachers can work together to gather any learning materials they need to take home.  This enables the child to be more successful upon return to CJA. CJA understands emergencies come up and that 24-hours notice might not be possible in an emergency situation.

How do I report significant medical history that needs to be shared at school (asthma, allergies, seizures, etc.)?

What if my child needs to take medication during the school day?

If it is determined that medication must be given to a student at school, the procedure below will be followed:

  • All prescription and over-the-counter medication will be administered by the school nurse. Students are not allowed to carry medications, with the following exceptions:
    • Students with Asthma may carry an inhaler. (Asthma Action Plan)
    • Students with IDDM may carry their insulin and supplies. (Diabetes Action Plan)
    • Students with severe allergies may carry an Epi-Pen. (Anaphylaxis Action Plan)
  • Health Emergency & Authorization Form must be completely filled out and signed by the student’s parent or guardian and licensed prescriber.
  • Prescription medication must be brought to the Nurse’s Office by a parent or guardian in the original container.
  • The following over-the-counter medications are stocked in the Nurse’s Office:  generic Advil, Tylenol, Benadryl and TUMS.
  • The School Nurse will dispose of any medications that are not picked up at the end of the school year.
